  • Support Form: This document confirms the accuracy of the adversity statement. The provider of this form must be at least 21 years old and well-acquainted with the applicant’s circumstances. Suitable individuals include neighbors, family friends, teachers, counselors, pastors, or coaches.

  • Proof of Citizenship: Applicants who were born outside the U.S. but are U.S. citizens must provide documentation such as a copy of their U.S. passport’s photo ID page, a citizenship certificate, or a parent’s citizenship certificate.

  • Financial Information: Since applicants must come from families with an adjusted gross income of $55,000 or less, supporting financial documents must be provided, varying based on individual circumstances.
